Filling in your request form:
Wherever possible the whole of this form should be completed with the young person present, if this is not possible, then make sure that their voice is included in appropriate sections of the form.
- Please make sure you take time to answer all the questions in as much detail as possible so that we get to know your young person a bit better. (Incomplete forms will be returned which may delay the start of mentoring)
- It’s especially important to let us know if the young person is already working with anyone else at the moment or if they have already tried other services, it is really helpful that we know that too.
- It really matters to us that we hear the young person’s voice. It’s okay for you to help them complete the form, but we need to know that they are invested in making this work for them.
- Every young person matters to us and it’s the person looking for mentoring we want to know about and hear from.
- If a young person is aged 13 or over they can self certify the data protection consent on the form. For young people under the age of 13 a signature from their parent / guardian will be required in order to process the form.
- Once you have sent us a completed form you should hear back within 2 weeks to set up a meeting to introduce the young person to their mentor. During this meeting we will get to know them better, set initial goals and make sure that mentoring is the right service for them.
